Prenatal Care Check off lists and charts

Are you preparing for a home birth?  Do you need some sort of check off list or method of organizing your healthcare into an easy to use format?  Are you even planning an unassisted home delivery?

If you answered "yes" to any of the above comments, I know that you will probably need some sort of record or organization sheets to help make your birth the best that it can be.  If you fill in these sheets, you will be able to communicate with health care professionals and they will be able to understand your health conditions.  Some of you who birth unassisted do not want to use any form of medical recording system and that is ok, but for the sake of those who are just curious and are "friendly" with the health profession, these sheets are for you.

I have delivered two children from the posting of this blog post unassisted.  I was "worried" I had a bad condition and needed to deliver at the hospital.  It turns out that I did not have the condition and was able to return to another location to deliver safely.  The sheets were good for communication with the health care workers to make coming to and checking out of the hospital easy.
